When an incident arises involving a chapter or member of Pi Alpha Phi causing bodily injury or property damage to any person, the following information must be obtained immediately. Some examples of reportable incidents include hazing, sexual misconduct, or fighting that results in injury. Do not withhold reporting an incident to obtain all required information. Because timeliness is of the essence, report it immediately and send a copy of the form below to nb@pialphaphi.com. This report can be submitted by both Pi Alpha Phi members and non-members.

The conduct process for sexual misconduct is separated from the undergraduate chapter leadership. Undergraduate members are expected to report any reports of sexual misconduct to Pi Alpha Phi National Fraternity using the INCIDENT REPORT FORM . The undergraduate chapter does not adjudicate reports with its own standards board.
